# US POINTER: a structured lifestyle program improved cognition

## The short version

A two-year US trial found that a coached lifestyle program improved cognition a bit more than the same advice given as a self-guided plan. Both groups improved, and the extra bump was small. It supports showing up with structure, not a claim that this protocol prevents Alzheimer's in APOE4 carriers.

US POINTER, the large American replication of Europe’s FINGER trial, reported in JAMA on July 28, 2025. FINGER is the Finnish Geriatric Intervention Study to Prevent Cognitive Impairment and Disability, a two-year multidomain lifestyle trial. POINTER was a 2-year randomized trial in **2,111** at-risk older adults (ages 60 to 79) testing a multidomain lifestyle program of exercise, a MIND-style diet, cognitive and social engagement, and regular vascular monitoring. Both arms got the program. The difference was how it was delivered: a **structured** arm with 38 facilitated team meetings and coaching, versus a **self-guided** arm with light encouragement and materials. Global cognition improved in both arms, and the structured arm improved more (an advantage of about 0.03 SD per year, p = .008). [\[1\]](/digest/us-pointer-trial-reports#ref-baker-2025) The headline is less the effect size than the design lesson: in this trial, the structured system produced a small additional cognitive benefit. Why this matters for a carrier: the trial deliberately enrolled the cardiometabolic-risk, family-history profile many APOE4 carriers share, so it speaks directly to the population that most wants to act. It does not establish that the same result applies specifically to APOE4 carriers because genotype was not the basis of this comparison. [\[1\]](/digest/us-pointer-trial-reports#ref-baker-2025)

The honest caveat: these are 2-year cognitive outcomes in people who were cognitively healthy at the start, and both arms were active, so this is structured versus self-guided, not lifestyle versus nothing.
