# Shift work, jet lag, and the body clock

## The short version

Protect sleep, use light on purpose, and keep meals and activity on a regular clock when shift work or travel knocks you off. Those steps help sleep and metabolism. They are not proven to prevent dementia in APOE4 carriers.

[Sleep is a real brain-health lever](/topics/sleep-and-apoe4), but here is the part most people miss: *when* you sleep matters too, not just how much. Your **circadian rhythm**, the roughly 24-hour internal clock, runs far more than drowsiness. It sets the timing for your metabolism, blood pressure, and the brain’s overnight cleanup. When shift work, jet lag, or even chronic “social jet lag” throws that clock off, it works against the exact systems an APOE4 carrier is trying to protect. The good news is that the clock takes its strongest cue from one thing you can control: light. This is how the clock works, what disruption costs, and how to defend it.

## What the body clock actually controls

Think of a conductor in your brain keeping the whole orchestra in time. A master clock, set largely by **light**, coordinates a daily rhythm across nearly every organ. As [NIGMS](https://www.nigms.nih.gov/education/fact-sheets/Pages/circadian-rhythms.aspx) describes, that rhythm governs sleep-wake timing, hormone release, body temperature, and **metabolism**.

When your behavior lines up with the conductor, meaning you sleep, eat, and see light at the expected times, the systems run smoothly. When you are chronically out of sync, things drift in the wrong direction: blood sugar control worsens, blood pressure patterns shift, and both sleep quality and the brain’s overnight [waste clearance](/topics/sleep-and-apoe4) suffer.

## Misalignment taxes metabolism, vessels, and deep sleep

Circadian misalignment is not a vague wellness concern. It has measurable costs, and they stack onto risks carriers already manage.

-   **The metabolic cost is real and fast.** In a tightly controlled lab study, Buxton 2012, just a few weeks of misaligned sleep and meals cut resting metabolic rate by about **8%** and pushed post-meal glucose up enough to look **prediabetic**. -   **The cardiovascular cost shows up over years.** Long-term shift work is associated in population studies with higher cardiovascular risk, which loops back to the [brain-heart axis](/topics/brain-heart-axis-vascular-cognitive-decline).
-   **The sleep-quality cost is the sneaky one.** Sleeping at the wrong circadian phase, like daytime sleep for a night worker, yields less deep slow-wave sleep even when the total hours are identical. So you can log a full 8 hours and still shortchange the glymphatic [waste clearance](/topics/sleep-and-apoe4) that depends on that deep stage. Timing, not just duration, drives the brain’s overnight housekeeping.

This is not a reason for shift workers to panic. Millions do this work, and the goal is to **reduce the damage**, not to quit your job. The tools below do most of that work.

## Protecting the clock as a shift worker

Light is your strongest lever, so use it deliberately. The key fact most people get wrong is the intensity gap: outdoor daylight is **10,000 to 100,000 lux**, while typical indoor light is only a few hundred. So “leaving a lamp on” barely registers to your clock. Real circadian signaling takes real brightness.

-   **Use bright light to stay alert, darkness to recover.** Get bright light (outdoors or a **10,000-lux light box**) during your work shift, then enforce darkness on the way home, wearing sunglasses after a night shift, so your clock can settle toward your sleep window.
-   **Make daytime sleep count.** Black-out curtains, an eye mask, earplugs or white noise, a cool room, and a “do not disturb” boundary so the household protects your sleep window.
-   **Anchor whatever you can.** Keep meal timing and your sleep schedule as consistent as your rotation allows. Wildly different patterns every day are the hardest thing on the clock.
-   **Front-load caffeine.** Use it early in the shift, not near the end, so it does not wreck your post-shift sleep (see [caffeine and sleep](/topics/coffee-tea-and-the-brain)).
-   **Nap smart** before or during long shifts when permitted (see [naps](/topics/naps-melatonin-and-sleep-aids)).

## Handling jet lag

Jet lag is the same problem in miniature: your clock is in one time zone and your body is in another. Eastward travel, which means advancing the clock, is generally harder than westward. The fix is to drag your clock to the new zone fast, and again light leads.

-   **Shift gradually before you go.** When you can, nudge your sleep and meal times toward the destination over the few days before travel.
-   **Chase or avoid light by direction.** Getting the right light at the right time is the most powerful reset tool there is. Apps like Timeshifter or Entrain build a personalized schedule for which hours to seek light and which to block.
-   **Time melatonin as a signal, not a sedative.** Roughly **0.5 to 3 mg taken at the destination’s bedtime**, on the travel day and for a few days after, has the best evidence for jet lag. This is one of melatonin’s most established uses. Clear it with a clinician.
-   **Adopt local time immediately** for meals and activity, and get outside in daylight as soon as you can.

## Light, meals, and a protected sleep window

-   **Treat light as your main tool.** Bright when you need to be awake, dark when you need to sleep, and remember that indoor light is too dim to count.
-   **Protect the daytime sleep window** as fiercely as a nighttime one, since it is more fragile.
-   **Anchor meals and schedule** as much as your rotation or trip allows.
-   **For travel, shift early, chase light by direction, and time low-dose melatonin** to the destination night.

## Common questions

**Is shift work bad for my brain?** Chronic circadian disruption carries metabolic and cardiovascular costs and shortchanges restorative sleep, so it is a real risk worth managing. But a deliberate light strategy and protected daytime sleep blunt much of it. The goal is mitigation, not alarm.

**What is “social jet lag”?** It is the mismatch between your weekday and weekend sleep schedules. Sleeping in for hours on weekends nudges your clock around like a mini trip across time zones, so a more consistent schedule helps even if you never work nights.

**Does melatonin fix jet lag?** It helps as a *timing signal* when used correctly, meaning a low dose timed to the destination night. That is one of its better-supported uses, unlike its weak role as a general sleeping pill.

> Your clock is one more system worth keeping aligned, and light is your most powerful tool for steering it. If you work nights or cross time zones, protect your sleep window and use light deliberately.
