# Ultra-processed food and the brain

## The short version

Cutting ultra-processed food is a reasonable way to support diet quality, blood pressure, lipids, and metabolic health. The reported cognitive link is not proof of cause, and it is not an APOE4-specific finding.

People who got more than a fifth of their daily calories from ultra-processed food showed about 28% faster decline in overall thinking ability than people who ate the least. That came from nearly 11,000 adults followed for eight years, and the threshold is not extreme. The average US adult already gets well over half their calories from this category. For an APOE4 carrier, the reason to care is specific: ultra-processed food pushes on exactly the systems the gene already strains. Here is what the research shows, why it likely matters, and the simple way to eat around it.

## What “ultra-processed” actually means

“Processed food” is a slippery phrase, since chopping a carrot is technically processing. The term researchers use is ultra-processed food, or UPF, and it comes from a classification system called NOVA that sorts food by how industrially transformed it is.

Ultra-processed means industrial formulations built largely from refined ingredients, additives, emulsifiers, and substances you would not have in your own kitchen, engineered to be hyper-palatable and shelf-stable. Think packaged snacks, sodas, sweetened cereals, reconstituted meats, instant noodles, and most of what lives in the center aisles. The useful test: if the ingredient list is long and full of things you do not recognize, it is probably UPF.

## What the research is finding

This research is younger than the diet-pattern work, but it is consistent. The 2023 study in *JAMA Neurology* followed nearly 11,000 Brazilian adults for around eight years. People who got more than 20% of their daily calories from UPF had roughly 28% faster decline in global cognition and about 25% faster decline in executive function (the planning-and-focus skills) than those who ate the least.

For scale, that 20% threshold is low. Ultra-processed foods make up an estimated 57 to 60% of calories in the average US adult’s diet, so most people are already well past it. Other research links heavy UPF intake to cardiovascular, metabolic, and weight problems, the very pathways that feed [brain risk](/topics/brain-heart-axis-vascular-cognitive-decline) in carriers.

The honest caveat: this is observational. People who eat a lot of UPF differ from those who do not in many ways, and untangling cause from correlation is hard. The NOVA system is also blunt. It scores by degree of processing, not nutrient content, so a plain whole-grain product and a candy bar can both land in the ultra-processed bucket. So treat this as a consistent, plausible signal, not a settled verdict.

## Why it likely matters, especially for carriers

You do not have to invoke a mysterious additive to explain the link. UPF tends to be calorie-dense, low in fiber, high in refined carbs, sodium, and unhealthy fats, and engineered to be easy to overeat. That combination pushes weight, blood sugar, blood pressure, and lipids in the wrong direction. There is also active research into whether specific emulsifiers (the additives that keep ingredients from separating) disturb the gut lining and drive inflammation, though that work is still early and mostly from animal studies.

For an APOE4 carrier, the problem gets pointed. UPF hits three of the gene’s particular vulnerabilities at once:

-   The **saturated fat** in many processed meats and snacks drives the [E4 LDL hyper-response](/topics/apoe4-and-saturated-fat) (LDL is low-density lipoprotein, whose particles can deposit cholesterol in artery walls), where carriers’ cholesterol jumps more than other people’s.
-   The **refined-carb load** spikes insulin, and chronically high insulin may further impair the amyloid clearance that E4 already does poorly.
-   The **whole package** worsens the vascular and metabolic risk carriers most need to manage.

So UPF is not just generically unhealthy for a carrier. It presses on the exact systems APOE4 already leaves under strain.

## The simple way to eat around it

The good news is you do not need to memorize a classification system. -   **Build meals from recognizable whole foods:** vegetables, legumes, whole grains, fruit, nuts, fish, olive oil.
-   **Cook a bit more** than you order or unwrap.
-   **Cut the obvious offenders first.** Sugary drinks and daily packaged snacks are the highest-yield swaps. If you do only one thing, drop the sugar-sweetened beverages.
-   **Read the ingredient list, not just the front of the box.** “Made with whole grains” on top of a long additive list is still UPF.
-   **Don’t aim for purity.** A 90%-whole-food week beats a “perfect” plan you abandon. The pattern is what counts.

## Common questions

**Is all processed food bad?** No. Canned beans, frozen vegetables, plain yogurt, and whole-grain bread are processed but not ultra-processed, and they are fine. The concern is specifically the heavily formulated, additive-rich category.

**What is the single best change to make?** For most people, cutting sugar-sweetened drinks. It is the highest-yield, lowest-effort swap.

**Do I need to count or track UPF?** No. Shifting your center of gravity toward whole foods does the work without tracking.

> Think of it as a center-of-gravity shift rather than a strict rule: let real food be the default, and let the ultra-processed stuff become the occasional exception.
